Regional Coordination Forms


Below are regional coordination forms for District Days 2011!  The forms are organized by geographic region and also provide additional background information on your legislator.  To use the form, find your county and determine which legislators represent your county.  The form also lists the other counties represented by the legislator so that you can coordinate with other folks for scheduling and attending district visits.

In addition to the counties represented by the legislators, these forms include other information about each legislator, including:

Signed PACT? – Has the legislator signed the PACT Coalition’s Resolution supporting the TEROC Master Plan?
Tobacco Money 2001-2010 – The total amount of campaign contributions the legislator has accepted from tobacco interests in this time period.
District Office Locations – Cities where the legislator has district offices (addresses and phone numbers available on legislator’s website).
People/Coalition Planning a District Visit – Write the names of the LLAs, county staff, competitive grantees and coalition members who will be participating in a district visit to the legislator.
